Sequoyah Youth Track & Field Club | 2010 Registration

REGISTRATION IS CLOSED!

We will be starting up a club out of River Ridge this year like we did several years ago with Creekview. River Ridge and Sequoyah will have all practices and events together but different uniforms on meet days.

Please download the form for your feeder high school and bring to registration. Along with the registration form, I will need one copy of birth certificate and a copy of insurance card.

2010 Registration Dates:

:: Thursday, Feb. 4th from 5:00pm-7:30pm
at the Hickory Flat Library (CURRENT MEMBERS ONLY)

:: Saturday, February 6th from 2:00pm-4:30pm
at the Hickory Flat Library (OPEN REGISTRATION)

Please Note: The number of athletes per age group will be limited.

Please confirm by email that there is space available. Then download registration form and mail that and all other information needed to: 1985 Settindown Drive, Roswell, GA 30075

Steps to Registering: (for Boys and Girls ages 6-18)

1. Download forms and fill them out
2. Must have 1 copy of birth certificate
3. Copy of Insurance
4. Uniform sizes (we will have samples at registration)
5. Fees: $100 includes uniform or if you use your uniform from last year it will be $90.
6. Must feed into either Sequoyah or River Ridge High School

Please Note: Athlete will not be allowed to practice until all the proper forms are turned in.
